It's frequent everywhere in the world. Institutional memory is key.

I would expect, as a government entity that funds a lot of IT modernization efforts and systems for agencies, I'm a little shocked that they don't have a way to pull up taxpayer-funded applications.

Well, it wouldn't be the application. It would be a specialized series of documents associated with P3CO review. Again, this would have been very early on, too, so the formalized structure for that review wasn't, I think, finalized until '16 or '17, I think, with the NSABB stuff. DC.Scheduling@LexitasLegal.com
