March 9, 2021 Dear i Enclosed please find my 2019 Tax Organizer together with the appropriate forms including W-2s, 1099R, 1099-G,1099-INT, 1099-DIV, 1099-MISC, 1099-SSA, Schedule H (Form 1040) for my household employee and the 1095 C forms for me and my wife (we did not receive a 1095 B form). Similar to last year, I understand that you and will be putting together the 1099-R for my Defined Benefit Plan. I have filled out all of the necessary information in the Organizer together with added comments. There are a few points some of which will be obvious from the Organizer, and other additional ones that I would like to bring to your attention: 1) 2) 4) As per last year, I am enclosing three signed Forms 8879-F for the e-file signature authorizations for Form 1041 for each of my children's Trust. They are not yet filled out (1 assume that you will do that), but I have signed them. Please let me know if I have done this correctly or if there is something else that I need to do with them. My miscellaneous (non-government) income for 2019 with regard to royalties from McGraw-Hill for my textbook was $308,436. (see enclosed 1099-MISC). I also received a $5000 prize from The Partnership for Public Service (see enclosed 1099-MISC). Please note that I will receive the Dan David prize from Israel in May 2021. This is a $1 million prize. One hundred thousand dollars of which will be directly given to charity and so I will receive a check for $900,000. Do we need to figure this in my estimated taxes for 2021? With regard to the 529 Plans, I have not drawn on these this year since none of my daughters had tuition this year.
