Likely start vaccinating within a few days (likely Monday, 21 December) Just found out that NIH will finally get doses of vaccine from Moderna (see above) which means I will likely get vaccinated on Tuesday, 22 December. I am insisting that I receive vaccine ONLY if Clinical Center ICU people ALSO receive it at the same time as I do, • Cases continue to surge. Deb's comments: Continued deterioration in CA, AZ, NV,TX, FL, PA, OH, NJ, NY and will need staffing support in most of these states except FL and TX - most concerning is CA and NV for ICUs and hospital staffing. States are now using their vents that are in their state stockpiles which is the first we have seen this occur since summer. There is significant diversity in approach to vaccination - many states approach are self defined co-morbidities which could lead to long lines if its not a combination of age plus - ie first group essential worker over 50 and general population over 65 combination of risk of exposure and risk of severe disease. New strain of virus - B.1.1.1.7 arising in UK with concern that it is spreading much faster that other strains. Some EU countries are banning flights from UK. The rate of rise is concerning, however due to aggressive mitigation there is a more rapid stabilization of cases then we saw in the Fall with the center of the country which should lead to an earlier peak if the Holiday Season surge can be blunted. Had Zoom call with Biden. Also, present were Ron Klain, Jeff Zientz, Rochelle Walensky; Natalie Quillian; Vivek Murthy. Discussion centered around: 1) state of the outbreak (me); 2) Mutants from UK (me); 3) vaccine rollout (Vivek). I strongly recommended not banning travel from UK. Ron Klain and Biden were not so sure. Ron suggested that we might have a 10 day ban over the holidays. I pushed back and said that this would not help since the virus would come from other places. I suggested that we very explicitly state that we are watching this very carefully and are all over it. The potential effects of the mutation is : 1) increased transmissibility; 2) increased virulence; 3) evasion of protection from vaccine (and to some extent from mAb). All of these are testable.
