October 28, 2020 - Global USA Went on rounds today with a patient with COVID-19 in the SCSU. When I entered the SCSU nurses' station, the group of 5 nurses screamed in delight that I had visited and we took a whole bunch of picture. Brett Rowland (security) was the photographer. White House still claiming that everything is under control and that all that matters is deaths. That is absurd. Deaths are in fact increasing; however, they are completely disregarding the morbidity associated with getting infected and not dying, which could be protracted. Here's A Timeline Of President Donald Trump's And Dr. Anthony Fauci's Relationship. USA Today (10/28, Behrmann, Santucci, 10.31M) reports, "The relationship between President Donald Trump and Dr. Anthony Fauci, the nation's leading infectious disease expert, has publicly eroded as the coronavirus pandemic has raged through the United States, with the two officials often disagreeing on issues from what drugs to use and a vaccine timeline to something as simple as the fact that COVID-19 isn't 'going to disappear.'" According to the article, "over the course of the virus' spread, Trump's relationship with Fauci, who has worked under both Democratic and Republican administrations since 1984, has vacillated from support to disdain." USA Today provides "a timeline of the pandemic, reports about their relationship, and public statements from the two men regarding the virus and each other." 8. 28 October 2020 We are now seeing both small and large metros in the previous hot spots in the spring and summer beginning to exhibit significant community spread and with cooling this spread could significant accelerate as in CHI and hit new alltime records, There is no evidence that previous experience with significant viral spread is in any manner protective of this fall surge Hospitalizations are rising in all state categories (red, orange, yellow, light green) except for dark green states at this time -- we are seeing record numbers of new admissions with no month over month improvement in thee fatality rate since July Fatality rates in people over 70 remains at 10-11% of all cases. Increased access to novel therapeutics will be needed to change this. Cases will cantinue to hit all time highs week over week as the core mitigation efforts and targeted testing for asymptomatic infection in those under are not occurring. (Last week we were up nearly 9K high over high and with current slope it means we could cross 85 90K the end of this week and 100K/day the end of the next week--- its not the absolute case number but these numbers are an indication of community spread and community spread leads inevitably to infection to those vulnerable to serious complications and deaths) This sequence of events must stop with the initial silent spread with aggressive sentinel surveillance testing. Aggressive testing of the contagious but asymptomatic population with isolation effectively contained and mitigated spread prior to infection of the more vulnerable community members. It is critical ta act now finding the continued source of silent spread, ensuring infected individuals know how to limit their infection of others and with messaging on universal masking, physical distancing, and essential limitation of public and private gatherings indoors especially small gatherings with friends and family. One unified consistent messaging about masks, physical distancing and hand washing with profound limitation on indoor gatherings especially with family and friends is essential at this time point -- this is about empowerment Americans with the knowledge and data for decision making. October 29, 2020 - Global - 44,000,000/1,200,000; USA - 8,800,000/227,000 POTUS in complete denial that there is a raging pandemic and everything is spiking. He keeps insisting that we have "turned the corner" or "rounding the turn". Some potentially nefarious stuff going on. The FDA career scientists are hesitant to grant EUAs to Lilly's and Regeneron's mAb based on the current data. Alex Azar is really pushing them and furious because he thinks FDA is "slowwalking" the process so as not to have it out before the election in 5 days. Clearly, Alex wants to please the POTUS so that they can say that they have a breakthrough (similar to Convalescent plasma nonsense) right before the election. Azar then asked NIH (Collins), CDC (Redfield) and ASPR (Kadlec) to weigh in since the legislation that established the EUA process (dating back to the Anthrax situation in 2001) requires the CDC, NIH, and ASPR Directors to weigh in on EUA decisions (even though they have not done so in the past). Cliff and I had long conversation with Francis on his way down to HHS to discuss with Azar. We encouraged Francis to support the career scientists at FDA since they do not want to rush this through. They need more time to gather important information since what they have now is rather weak.

Cases will continue to hit all time highs week over week because the core mitigation efforts and targeted testing for asymptomatic infection are not occurring resulting in continued and escalating community spread leading to increasing hospitalizations and fatalities, States have developed testing systems and contact tracing for symptomatic clients that may account for less than 50% of the community spread as sick individuals have less activity in the community. Those with such mild symptoms or asymptomatic remain in the community for their full infectious period accounting for more of the community spread. Strategic use of the Ag tests will be critical to contral the current surge. These mild and asymptomatic cases will require both sentinel surveillance of community populations (community college students, major industries, K-12 teachers etc) as well as routine testing of under 40 yo to find and isolate these contagious individuals, The sequence of events must stop with the initial silent spread with aggressive sentinel surveillance testing, Aggressive testing of the contagious but asymptomatic population with isolation effectively contained and mitigated spread prior to infection of the more vulnerable community members. Anthony S. Fauci
Oct. 30, 2020 at 8:00 a.m. EDT Months before travel was hobbled by coronavirus fears, I was returning home one evening from Raleigh-Durham International Airport when I noticed a familiar figure at the gate -- slight, bespectacled, hair side-parted and gray. Anthony S. Fauci and I have spoken now and then over the years, and we began to chat. When it was time to board, it turned out our seats were adjacent. So, I was treated to vivid war stories about the arc of his medical and scientific work in infectious diseases, and about his interactions with a succession of presidents during the AIDS crisis. New York Times - 10/29/2020 - Thomas Kaplan: Mr. Biden has also signaled that Dr. Anthony S. Fauci, the government's top infectious disease expert, will have a prominent role in a Biden administration. The Biden campaign has said Dr. Fauci, the longtime head of the National Institute of Allergy and Infectious Diseases, "will have full access to the Oval Office and an uncensored platform to speak directly to the American people -- whether delivering good news or bad." -- Thomas Kaplan and Abby Goodnough

November 1, 2020 - Global - 45,900,000/1,200,000; USA - 9,110,000/230,000 Big ISSUE!! - Blockbuster situation in Washington Post in an interview that I gave on Friday evening to Josh Dawsey and Jasmeen Abutaleb. I spoke about the fact that cases were dramatically increasing throughout the country (see prior days above) and that we needed to do something different like doubling down on public health measures. I also strongly blasted Scott Atlas. In the article, much to my dismay, Dawsey (less than Abutaleb) wanted to keep the word "seriously in a passage that I thought was off the record, but he said that it was not (I am pissed at him). Here is the passage: Fauci said former vice president Joe Biden's campaign "is taking it seriously from a public health perspective." Trump, Fauci said, is "looking at it from a different perspective." He said that perspective was "the economy and reopening the country.". Also, in the article. Judd Deere blasted me saying I was being political: Judd Deere, a White House spokesman, offered blistering criticism of Fauci for his comments in a statement to The Washington Post on Saturday. Deere said Fauci "knows the risks [from the coronavirus] today are dramatically lower than they were only a few months ago." "It's unacceptable and breaking with all norms for Dr. Fauci, a senior member of the President's Coronavirus Task Force and someone who has praised President Trump's actions throughout this pandemic, to choose three days before an election to play politics," Deere said. "As a member of the Task Force, Dr. Fauci has a duty to express concerns or push for a change in strategy, but he's not done that, instead choosing to criticize the President in the media and make his political leanings known by praising the President's opponent -- exactly what the American people have come to expect from The Swamp." I thought that I was in big trouble. However, some interesting things happened. Alyssa Farah texted me at 7:00 AM on Sunday and said: I am so sorry that fool Scott Atlas keeps attacking you. I'm even more sorry I'm not in a better position of influence to see him out the door. I'm trying to make people see he's a disaster. I immediately called her up and explained that I was not pitting my support for Biden over the POTUS. She understood perfectly and said she totally supported me and thanked me for what I do. Then, at 9:30 AM, the POTUS called me from Air Force One. We had a 15 minute conversation where he did his usual rambling. He started by saying that I really need to be positive. "Tony, I really like you, and you know that, but what the fuck are you doing? You constantly drop bombs on me. Everybody wants me to fire you, but I am not going to fire you, you have too illustrious a career, but you have to be positive. The country cannot stay locked down. (I never said that we should lock down). You have got to give them hope. I like you but so many people, not only in the White House, but throughout the country hate you because of what you are doing. I am going to win this fucking election by a landslide. Just watch. I always did things my way. And I always win, no matter what all these other fucking people think. And that fucker Biden. He is so fucking stupid. You are 10 times smarter than he is. I am going to kick his fucking ass in this election. I will win by a landslide. Just wait and see. I will win Florida, Pennsylvania, Ohio, all the key states. Just wait and see. The smallest crowd that I had was 25,000 people. Biden had 5 people blowing their fucking horns in their cars. Tony, you have to be positive. I do not want to keep you off TV, but when you are out there, you have to be positive. (I tried to interject some words that I am being positive by telling people what they need to do to avoid infection - he heard none of it and just kept talking over me. He did not seem hostile, but he was just rambling). Then when he was finished, he said: "OK, Tony, I will see you in a couple of days. Take care". Although I do not think that I will be fired, I am not out of danger just yet. Another crazy thing. Scott Atlas did an interview from the White House with Russian Television blasting the response of the USA scientists (including me). CNN is jumping all over this.
