October 23, 2020 Global - 41,600,000/1,100,000; USA - 8,400,000/222,400 Last night was final Presidential Debate. POTUS still insisting that we are turning the corner and all parameters are down. This is completely untrue as per the summary below from Deb Birx. Also, after the moderator brought up the fact that POTUS called me and others "idiots" he continues to say that I am a nice guy Confirmed Cases [Thousancs) So Bo So a0 zo io New Daily Confirmed COVID-19 Cases in the U.S. Dh Day eee Day Mowing Avarage E.5 but have been wrong many times. He brings up the "incomplete" quote where I say that: ".....at this time, we do not need to do anything different", yet he leaves out the second part of the sentence: "......however, it really is an evolving situation, and we have to be prepared for the worst". This was a quote from me on CNN on January 20, 2020. On Jan. 21, the 1st travel-related case was detected in the USA. On Jan. 24 the 2nd travel-related case was detected and on Jan. 30 the 1st person-to-person spread of SARS-C0V-2 was confirmed in the USA. Therefore, I was warning that we needed to be prepared for the worst even before the 1st case was detected in the USA. Also, he said again that I am a Democrat and brings up my early (and very brief) statement on masks. Fact checkers showed he lied dozens and dozens of times during the debate. • Did Fireside Chat with the Aspen Group - Sibelius, Thompson. Very well received. Had dinner at my home with Jake and Jennifer Tapper. Relaxed, good time. Jake clearly believes that Trump is totally nuts. Hit 83,000 new cases today. The single day record since the beginning of the outbreak: Surge continues: 1. 2. 3. 4, 22 October 2020 Rapid viral spread in both urban and rural counties continues and these new cases are filling hospitals in urban and rural areas making load balancing difficult significantly straining the hospital system -- we are 1-2 weeks from escalating fatalities. It will be critical to ensure that the North East, CA, TX, FL hold during this surge but there are early upticks across TX, FL metrosand across the Sunbelt, All-time record high numbers of cases expected tomorrow or next week and test positivity and hospitalizations continue to rise making it clear there is diffuse community spread and not testing -- current testing and Ag tests have not been aligned to detect the silent spread as universities have done -- Universities with routine required weekly testing has allowed universities to find the asymptomatic cases and isolated those cases quickly and dramatically decrease "community" spread among students -- the same approach can work at the community level... Across the North East and upper Midwest previously stable cities are seeing increases, Philly, CHI, Detroit. Daily hospital admissions are higher than summer peak (summer surge lead to 100K fatalities). Fatalities are increasing in IL, QH, PA, WI, MN This is not due to increased testing (many states have decreased testing rates) but broad and ever increasing community spread. There are no states, regions, or communities with enough priar COVID infection to provide any substantial protection fram new community spread which will lead to hospitalizations and fatalities. This is very evident by the rising cases again across the Sunbelt and moving inta the North East. This sequence of events must stop with the initial silent spread with aggressive sentinel surveillance testing. Aggressive testing in the contagious but asymptomatic population with isolation effectively contained and mitigated prior to infection of the more vulnerable community members. Providing appropriate isolation options for infected individuals from multigeneration households will be key. We have found from careful analysis, modelling and triangulation of all the data that community spread can be controlled without a lockdown as we have done but the time to act is critical and must be with the original detection of the silent spread, ensuring infected individuals know how ta limit their infection of others and with messaging on universal masking, physical distancing, and essential limitation of public and private gatherings indoors especially small gatherings with friends and family. Thankfully, they weighed heavily on keeping the trial going and not unblinding it after the first sign of efficacy, which would negate the further gathering of important data. The Australians had zero new infections in the entire city of Melbourne yesterday proving that pure public health measures such a mandatory mask wearing ($1000.00 fine) and avoiding crowds actually works. Alyssa Farah and Roma Daravi (White House comms) are continuing to jerk me around regarding clearances for TV requests. I get 20 requests and they ignore them. We complain, and then Alyssa says that she has no problem with any of them. "Go ahead and work with Roma to schedule". Roma gives us one or two and then we do not hear from them (ignore us) for another week. All the while they are approving Azar and other docs on the task force (Hahn, Redfield, Adams, etc.) Clearly, they do not want me on Major TV, but are afraid to admit it. This must be pointed out in the book! PRESS: WAMU podcast with Diane Rehm; Carnegie Endowment for International Peace - Fireside Chat; Rolling Stone Interview with Jeff Goodell Global USA sident

October 28, 2020 - Global USA Went on rounds today with a patient with COVID-19 in the SCSU. When I entered the SCSU nurses' station, the group of 5 nurses screamed in delight that I had visited and we took a whole bunch of picture. Brett Rowland (security) was the photographer. White House still claiming that everything is under control and that all that matters is deaths. That is absurd. Deaths are in fact increasing; however, they are completely disregarding the morbidity associated with getting infected and not dying, which could be protracted. Here's A Timeline Of President Donald Trump's And Dr. Anthony Fauci's Relationship. USA Today (10/28, Behrmann, Santucci, 10.31M) reports, "The relationship between President Donald Trump and Dr. Anthony Fauci, the nation's leading infectious disease expert, has publicly eroded as the coronavirus pandemic has raged through the United States, with the two officials often disagreeing on issues from what drugs to use and a vaccine timeline to something as simple as the fact that COVID-19 isn't 'going to disappear.'" According to the article, "over the course of the virus' spread, Trump's relationship with Fauci, who has worked under both Democratic and Republican administrations since 1984, has vacillated from support to disdain." USA Today provides "a timeline of the pandemic, reports about their relationship, and public statements from the two men regarding the virus and each other." 8. 28 October 2020 We are now seeing both small and large metros in the previous hot spots in the spring and summer beginning to exhibit significant community spread and with cooling this spread could significant accelerate as in CHI and hit new alltime records, There is no evidence that previous experience with significant viral spread is in any manner protective of this fall surge Hospitalizations are rising in all state categories (red, orange, yellow, light green) except for dark green states at this time -- we are seeing record numbers of new admissions with no month over month improvement in thee fatality rate since July Fatality rates in people over 70 remains at 10-11% of all cases. Increased access to novel therapeutics will be needed to change this. Cases will cantinue to hit all time highs week over week as the core mitigation efforts and targeted testing for asymptomatic infection in those under are not occurring. (Last week we were up nearly 9K high over high and with current slope it means we could cross 85 90K the end of this week and 100K/day the end of the next week--- its not the absolute case number but these numbers are an indication of community spread and community spread leads inevitably to infection to those vulnerable to serious complications and deaths) This sequence of events must stop with the initial silent spread with aggressive sentinel surveillance testing. Aggressive testing of the contagious but asymptomatic population with isolation effectively contained and mitigated spread prior to infection of the more vulnerable community members. It is critical ta act now finding the continued source of silent spread, ensuring infected individuals know how to limit their infection of others and with messaging on universal masking, physical distancing, and essential limitation of public and private gatherings indoors especially small gatherings with friends and family. One unified consistent messaging about masks, physical distancing and hand washing with profound limitation on indoor gatherings especially with family and friends is essential at this time point -- this is about empowerment Americans with the knowledge and data for decision making. States will begin to react with the spring mitigation efforts but this won't control the indoor house spread. . . October 29, 2020 - Global - 44,000,000/1,200,000; USA - 8,800,000/227,000 POTUS in complete denial that there is a raging pandemic and everything is spiking. He keeps insisting that we have "turned the corner" or "rounding the turn". Some potentially nefarious stuff going on. The FDA career scientists are hesitant to grant EUAs to Lilly's and Regeneron's mAb based on the current data. Alex Azar is really pushing them and furious because he thinks FDA is "slowwalking" the process so as not to have it out before the election in 5 days. Clearly, Alex wants to please the POTUS so that they can say that they have a breakthrough (similar to Convalescent plasma nonsense) right before the election. Azar then asked NIH (Collins), CDC (Redfield) and ASPR (Kadlec) to weigh in since the legislation that established the EUA process (dating back to the Anthrax situation in 2001) requires the CDC, NIH, and ASPR Directors to weigh in on EUA decisions (even though they have not done so in the past). Cliff and I had long conversation with Francis on his way down to HHS to discuss with Azar. We encouraged Francis to support the career scientists at FDA since they do not want to rush this through. They need more time to gather important information since what they have now is rather weak.
