I have copied Wayne and Martin so they can get you the financial gift fund information. Carl (not sure about DMID with Emily) can provide project information. JJ

Sec. Azar and I are supposed to meet with the POTUS in the Oval on Tuesday at 1:30 PM, but there is no information about the agenda. Apparently POTUS asked for this meeting to be set up, but no one in the WH seems to know why. Alex is trying to guess what this might be about; one possibility might be the universal flu vaccine, since Bill Gates recently spoke to the POTUS about that. See https://www.statnews.com/2018/04/30/bill-gates-vaccine-trump-meeting/ for the report on this conversation. (By the way, I'm surprised that Bill seems to be promoting this idea of the universal flu vaccine as if the US government hasn't been properly focused on it -- surely he knows about all the work that NIH has been doing, and about the current strategic plan? And how could his promised $12M add much to the much larger investment NIAID is making? Is this coordinated in some way, or is Bill going rogue?) Alex indicated that Gates had apparently told the POTUS that $5-6 billion was really needed to prepare for a worldwide flu pandemic. I'm not sure where that number comes from. The specific question that Alex thought might come up on Tuesday is what the funding trajectory is that's really needed if the preparation of the universal flu vaccine was to be considered the highest priority for the nation. What's the answer to that? Glad to speak by phone if that would make this easier. I Francis PS. Can I borrow your 3D printed model of the hemagglutinin protein, in case that turns out to be relevant?

Is this coordinated in some way, or is Bill going rogue?) The vaccine regimen is based on "mosaic" immunogens -- vaccine components designed to induce immune responses against a wide variety of global HIV strains. • Evaluation of a novel, long acting injectable antiviral for HIV Prevention o In November 2017, launched Phase III efficacy study (HPTN 084) to compare long-acting cabotegravir to daily Truvada for pre-exposure prophylaxis in HIV-uninfected women. • TB Drug development and clinical endpoints o NIAID intramural collaborations with BMGF have contributed to novel clinical markers for treatment response (PET/CT, NexGenNCT02371681) that are now further evaluated in the "PREDICT TB" trial (NCT02821832) to identify patients who may benefit from shortened therapy. o NIAID intramural, in collaboration with pharmaceutical partners (GSK, Merck) through the BMGF funded "TB Drug Accelerator" are co-developing novel anti-TB agents that are now entering clinical evaluation. o NIAID extramural programs are continuing to contribute to the establishment of best practices and preclinical/clinical evaluation of new drug candidates that are advanced through BMGF funded PDPs. • Single-dose HPV vaccine o In early November, NCI participated in high-level meetings at the Gates Foundation to discuss ongoing efforts to evaluate single-dose protection of the HPV vaccines. o If the hypothesis that one dose can confer strong protection is verified by the results of the trial, these results should enable this approach to be the standard of care and greatly increase HPV vaccination uptake - which currently stands at around 3% of eligible girls in LMICs - throughout the world. o In late November, the new Costa Rica Vaccine Trial ("ESCUDDO") started-- the first girls have now been immunized. o Additional studies planned to complement Costa Rica Trial; one immunogenicity study built to harmonize with the Costa Rica trial has already been initiated. • Contraception o A comprehensive analysis of publicly available gene expression and protein expression data has been performed to identify reproductive-tract specific transcripts/proteins to serve as potential non-hormonal targets for male and/or female contraceptive development. o A web-based contraceptive target database containing all pertinent information is in development for public access with a beta version released early to mid-2018. o Results from early collaborations in two Grand Challenges Explorations topics related to contraceptive research and development for either men or women anticipated in 2018 and 2019. ▪ Successful projects may be integrated into a small, focused program ▪ Research resulting from these awards is expected to have a large positive impact on the quality of applications to the NIH in the near-term future • Indoor air pollution o Jointly launched RCT of liquefied petroleum gas (LPG) cookstoves and fuel in four LMICs (India, Rwanda, Guatemala and Peru). o First large-scale trial to investigate whether home use of cookstoves that run on LPG improve air quality sufficiently to offer measurable health benefits. o Enrollment began February 1, 2018. o Increased BMGF interest in endpoint of severe pneumonia confirmed by the study team with consolidations in the lungs and/or hypoxemia is leading to potential new designs, including proceeding with the trial as initially proposed, and in parallel, developing local pneumonia surveillance systems in the same localities. Hi Dexter and Belinda, Can you let us know: 1. How much we have remaining for MAL-ED funds? 2. How much will be spent in all of FY '18? 3. Will there be any funds left/carried over into FY '19? 4. Confirm that no NIH funds spent on MAL-ED in FY '18? (Can ignore FTE staff salary and travel.) Thanks, PK

Hi Francis, I did review and the only additions I offered were the HPV post licensure vaccine trial and the new African postdoctoral fellowship program, to bring more African trainees to our intramural labs. The group relied on the Gates Foundation awards data base to identify the Foundation's publicly accessible funding commitments for joint projects with NIH. Also, I'm digging out info on the Gates contribution to the WorldReport database maintenance (although this would be modest expenditure compared to the collaborative projects Larry has outlined) The Gates Foundation and ICs have made parallel investments surrounding some of the NIH/BMGF working group initiatives, but the funding is not easily sourced (e.g., the contraceptive targets database). We would need to confer with ICs to assemble reliable estimates Based on the intent of the request, I would be happy to provide any additional info that might be of use in your response. As one example, NIH and the Gates Foundation have ‎cooperated in the development of such transformative global health interventions as MenAfriVac and the rapid TB diagnostic- Xpert MTB/RIF. NIH cooperation with the Foundation extends to eleven ICs, etc Sent from my BlackBerry 10 smartphone.
