July 30, 2022 (Saturday) Just got a call at 12 noon from Kevin O'Connor that the president has had a rebound with a positive antigen test today. He is asymptomatic. This is the real problem since there are no guidelines from the CDC which dictate what to do when you have someone who you are testing virtually every day. The CDC health advisory on what to do following a rebound after Paxlovid treatment does not mention testing at all. the duration of isolation is based purely on symptoms and time from resolution of symptoms. Here again it goes back to my objection many months ago of the guidelines the CDC put out about reentry into society following infection. I had insisted back then that even though it might not be practical for everyone to get testing and often people need to get back to work after five days, they still should have put some guidance in for people who can get tested period now we are flying in the dark. Ron Klain is going to want him out as soon as possible; however, given that he is known to now be antigen positive he has to isolate for five days and then the question will arise of how often to test him and if he is positive should he be let out even with a mask. Cliff Lane feels strongly that he should be in isolation so long as he is antigen positive and can go out once he has two negative tests in a row. Got a call from Ashish making sure that we are on the same page. Also, he asked me if we should start a 2nd course of Paxlovid.     July 31, 2022 I gave the special session plenary lecture at the International AIDS Society (IAS) meeting in Montreal via a virtual presentation. I have given a plenary lecture and virtually every one of the IAS meetings since 1984. I reflected on what has transpired since the last meeting in Montreal which was in 1989. My presentation triggered a prolonged standing ovation from the audience. The session was chaired and moderated by Laura Waters of the UK. The president remains antigen positive for the second day in a row following his rebound. His CT values are 16 to 18. He has had no additional symptoms although he still has somewhat of a lingering cough. The plan is to keep him isolated until he asked two negative antigen tests in a row.     August 1, 2022 I have been somewhat concerned about Monkeypox the very beginning, but I am getting more and more concerned about it as time goes by. It clearly is spreading rapidly throughout the world the community of MSM with an estimated doubling time of about seven days; my concern is that we are all seeing some spray into the non-MSM communities such as the recent is very much bothering me is the continued plans to have MSM and gay pride galas at the same time that we are having acceleration of COVID as well as outbreaks of monkeypox (see above my conversation with Jen Ashton) I got my usual follow up call from Kevin O'Connor regarding the President's condition. He is concerned that his viral load is going in the wrong direction, namely, his CT value went from 18/16 to 16/14. He called me together with Walter Reed ID physician Wes Campbell to see if I agreed with them that they should not restart Paxlovid since the president does not have any new symptoms and his oxygen saturation is 98%. His chest is clear despite the fact that he has his typical nagging cough that antedated his illness. I agreed with them that they should not start a second course of Paxlovid. The administration will announce tomorrow that they are appointing a monkey pox Coordinator (Bob Fenton) and a Deputy Coordinator (Demetre Daskalakis). Clearly the response or at least the perception of the response of the government to monkeypox has not been good. They are hoping to reinvigorate the response by this appointment. Tonight, Kevin Munoz (WH Comms) called me and he asked me if it was OK to put a quote in the press release supporting this decision. The White House feels that because of my street credibility with the gay community that this would make the choice more acceptable. I agreed and provided a brief quote. Of Microbiology, Rutgers), Kevin Esvelt, (Assistant Professor of Media Arts and Sciences MIT Media Lab) and Steven Quay (CEO, Atossa Theapeutics) all totally non-credible. Angel Rasmussen is blasting them on twitter as hacks. Once per year the minority are allowed to call a hearing and so Paul et al are using this as a publicity stunt. The majority has not even showed up. It is a total farse. I got a follow-up from Chase Crawford from my legislative staff who listened to the hearing and the result was expected in which Ebright came out strong saying that we did do gain of function and that I lied Quay also said that. However Esvelt said that some may call this gain of function whereas other scientists would say it's not since gain of function is such an ill defined term. He came the closest to reality.
