First and foremost our Fall surge slope is nearly identical to the UK slope suggesting we have had our own more transmissible strain since then and it was responsible for the Fall surge in the Northern Plain states. My concern then was this type of surge moving into the more populous states as it has now in CA and now Tx and the sunbelt is unstable as well as the midAtlantic and NorthEast. Viruses are always in search of escape mutants than are more transmissible yet preserving replication capacity. This is happening around the globe and the immunologic pressure on viruses drive this potential and we continue to increase the immunologic pressure through percent of population infected, use of plasma and monoclonal Ab and now immunization. Key to RNA viruses with this of high mutation potential is to rapidly immunize the population especially those most likely to have high replication potential - those over 65. We have to accelerate the immunization of those over 65 ASAP. We can't hold back on the rest of Tier 1 awaiting 1a. We have been in 1a for a month without adequate progress. Biden is much more in favor of federal presence than is the Trump administration. Dan Lahart of Regis) Made rounds on COVID-19 patient at Clinical Center. I have been doing this about 1X per week. Saw patient today with COVID disease - 73 y.o. AA male, ex-marine, ex-State Trooper. Says this is worse he has ever felt. • My group (Mascola, Graham, Lane, Marston briefed the ART team led by Lu Borio and Marcella Nunez-Smith and David Kessler. We spoke about the mutant viruses in UK and RSA and their impact on mAbs and vaccines. 1. There is a significant increase in test positivity rate and a high hospital admission rate/case across all age groups 2. I have included the top 10 states for cases and it is a mix of states that were surging prior to the Holidays NY, TX, FL, GA, AZ and states that were improving or stabilzing prior to the Holidays CA, PA, OH, IL, NJ and tracking these states will be key to estimating the impact of the Holidays on cases and eventually hospitalizations The Fall surge in the Northern Plain states was more aggressive and we warned if this type of surge hit populous states this would threaten the health care system - this is what we are seeing in CA and to be clear this is spreading across the Sunbelt and into the North East. Slow start in the implementation of administration of vaccines. Was supposed to have 20 million doses distributed and administered by December 31. Today there have been 15 million distributed and 5 million administered. January 8, 2021 - Global USA 4111 Deaths!! Record hospitalizations. I have been on the phone with Jeff Zients a few times today. I checked the number for projected requirement of vaccines to fulfill the promise of 100 million doses in arms in 1st 100 days of Biden administration. The number in the Biden slides (Kessler derived) was 275 million doses and 135 million people (getting 2 doses) by end of April. I called Moncef Slaoui and got the actual numbers and they totaled 189 million. I called Jeff and let him know. He checked with Kessler who then checked with Moncef and they figures that the doses that had already been allocated prior but not given yet would make up for the difference. I am concerned that we (Biden team) will not meet the promise. Too many vicissitudes in vaccine production. Perhaps Janssen will bale us out with the 25 million doses in March and 40 million in April (single dose required). Then, Biden team announced that they would not hold back any doses to save for 2nd shot, but will immunize everyone then can with doses they have. This places a lot of faith in the precision of the production/delivery of doses. They may have to pause with 1st doses to catch up with 2nd dose requirement. At 8:00 PM tonight as I was over Rob and Ellen's "open Garage" dining room, Jeff Zients called me again asking for a favor. A Politico reporter is already getting ready to write a story spreading rumors that Biden is upset with his COVID team and Jeff is asking a favor for me to go on "background" and tell the reporter that they are doing a very good job. I told Jeff that I would of course be happy to do this.
